SUPPORTED LIVING

We offer in Supported Living

  • 24-hour staffing by certified nursing assistants, including – but not limited to – ongoing health monitoring and nursing assessments
  • Assistance with daily living, including bathing, dressing, personal hygiene, grooming and transport to and from activities and dining
  • Medication reminders, assistance, and set-up
  • Three restaurant-style meals and snacks daily
  • Nutrition assistance and prescribed therapeutic diets
  • Engaging social and recreational activities and events
  • On-site beauty/barber services
  • Housekeeping and laundry services
  • Home maintenance
  • Health promotion and exercise programmes

Types of Support

In our supported living service, we will give you support and care plan that will specify the type of support you need in different areas of your life. Maintaining your home and managing your personal finances. Developing domestic and your life skills. We will help you to access employment, training or volunteering opportunities, developing and keeping personal relationship. We will make sure that you keep in touch with family. You will be offered an accommodation and fully assessed for its suitability in meeting the needs of residents. Our home will be accessible, safe and hazard-free. We will only allocate staff that have had their DBS certificate and safe to work with you. We also have staff that have experience working with vulnerable clients. All of our staff are getting comprehensive training to provide excellent care service.

DISABILITY

Our flexible and varied range of services providing disability support include: supported housing, supported housing with care, respite care and nursing or residential homes. Support time can vary from a few hours a week, up to full-time support or intensive support over a short period of time to help achieve a goal.

We provide expert tailored support and care for people aged 18 or over with mild, moderate or severe disabilities, including:

  • Autism
  • Sensory disabilities
  • Physical disabilities
  • Acquired brain injury
  • Associated mental health needs
  • Behavioural challenges

MENTAL HEALTH

We understand that suitable housing with the right support is an essential part of overcoming mental health challenges. Our range of mental health support services include: supported housing, supported housing with care, respite care and nursing or residential homes, community support and floating support.

The amount of support we provide varies depending on needs, and can be anything from a few hours a week, up to full-time support or intensive support over a short period of time to help achieve a goal. We also encourage peer support as much as possible, where mutual support and a shared journey empowers people to make the changes and learn together.

Damentia care- Nubah Social Care

Mental health is something that we can all relate to and something we provide support for in all services. Specialist mental health services support people with a diagnosed mental health condition, which can be anything from:

  • Anxiety and panic disorders
  • Bipolar disorder
  • Depression
  • Personality disorders

Through self-development and collaborative support with staff, the people we support gain self-confidence and improved emotional wellbeing on their pathway to independence and recovery.

HOMELESSNESS

We support adults who have experienced or are at risk of homelessness, and who may also need to overcome a dependence on addictive substances. We do this in a number of ways with:

  • Supported housing
  • Preventative outreach work
  • Resettlement support
  • Floating support services

YOUNG PEOPLE

We support young people aged between 16 and 25 by encouraging them to focus on their strengths and build positive support networks to achieve their aspirations, whether that is returning to education, finding meaningful work or making new friendships.

We specialise in supporting young people facing homelessness, including those who are:

  • Care leavers or looked after children (over 16)
  • Overcoming addiction
  • Young parents
  • Experiencing family or relationship breakdown

Our services for young people include supported housing services and three Foyers, which provide a safe place for young people to live, learn and make the transition to independent adulthood.

We also offer move-on services to help residents find and maintain their own independent accommodation. We also provide detached support or community outreach services where we provide one-to-one or group support to young people in the community (who don’t live in our properties).

DOMESTIC ABUSE

Domestic abuse can happen to anyone. It is defined as any incident or pattern of controlling, coercive, threatening, physical violence, threatening behaviour or abusive behaviour between intimate partners or family members, regardless of gender or sexuality.

We offer domestic abuse support services, providing support, advice and protection. If you have suffered any kind of abuse as below, we will help, support and protect you. One way we will help you is by providing basic counselling services.

  • Domestic violence or abuse.
  • Sexual abuse.
  • Psychological or emotional abuse.
  • Financial or material abuse.
  • Physical abuse.
